The nurse assigned at your first visit will be your nurse for every visit, unless you request a change. Unlike other agencies, we don’t change your caregiver because we believe in building personal relationships with all our patients.

What is Hospice/Palliative Care?

A philosophy of care centered around those with life-limiting illness and their family. Our care focuses on the whole person — mind, body, and soul. We strive to ensure all of our patients and their family care needs are addressed personally by their assigned case manager. Our Hospice patients are assigned a Personal Case Manager that can be reached 24/7 regardless of the time or day.
We cannot reiterate the importance of end of- life care, and consider it a great honor and privilege to be trusted by our Hospice patients and their loved ones to be a part of their life and care during this time. Our goal is to ensure that all patient care and comfort measures are addressed by our qualified Hospice Care team, so that your loved ones can focus on spending more time with you and have a piece of mind that every need that shall arise will be immediately addressed by our care team.

Who might need Hospice or Palliative Care?

Those diagnosed with a life-limiting condition with a prognosis of six months or less, should the disease follow its natural course or those with illnesses in which curative measures are no longer effective, appropriate or desired.
